Dr. Mathy can provide in-person psychological and neuropsychological services in the State of Wisconsin (License 4007).
VIRTUAL EVALUATION SERVICES - Certain services, including FAA ADHD Fast Track evaluations, can be conducted virtually. Dr. Mathy is licensed to provide virtual psychological and neuropsychological services to individuals in any of the following states (APIT Number 18569):

Adult Guardianship Evaluations
Pursuing a guardianship for a loved one is a challenging, but sometimes necessary process. Dr. Mathy is familiar with this process in the State of Wisconsin and can help make your evaluation as seamless as possible. She specializes in guardianship evaluations related to developmental disorders, neurodegenerative conditions, and/or severe and persistent mental illness resulting in deficits of cognitive functioning limiting a persons' ability to care for themselves and make sound decisions.
FAA Evaluations
Pilots, pilot candidates, or air traffic controllers may be referred for an aeromedical psychological or neuropsychological evaluation by the Federal Aviation Administration (FAA). The process typically begins by receiving a letter from the FAA dictating the need for additional information and evaluation. Dr. Mathy will work with you to meet your evaluation needs and ensures transparency in the process. Please complete an FAA Form 8065-2 for release of records to Dr. Mathy prior to your evaluation.
Fit for Duty Evaluations
Attempting to return to work following a significant illness, injury, or event may involve the need for a neuropsychological or psychological evaluation to assess your capacity to perform. Dr. Mathy conducts these evaluations for individuals recovering from brain injury/illness, and those who have concerns related to cognitive functioning. We can work with your referring agency or employer to ensure your evaluation provides the necessary information to advise your return to work process.
Psychoeducation & Consultation Services
Dr. Mathy taps into her passion for teaching when providing accessible psychoeducation services to audiences of community members, employees, and/or professionals. She maintains current knowledge and competence in the instruction of information related to ethical practice of psychology, interpersonal violence, the impact of brain injury/neurodegenerative disorders, and the psychological implications of death & dying.
Dr. Mathy also provides consultation related to implementation and provision of neuropsychological and evaluative services in clinical, health, and community settings.
